Hae dies

I'm searching for the composer of this piece of music. The only copy (see attachment) I have has been copied so many times that it's impossible for me to identify a composer name. I'm hoping that someone is familiar with the piece or recognizes it. My father used to play this piece every Easter. He is now deceased and I don't know where he got it from. The congregation loves it and when I play it now I always get questions about who composed it and where did you get that piece from. If I can find out the composer and publisher I would like to order new copies for my choir.

I've included a scan of the music. It's not great but it's the closest I have to the original! Please advise me as to maybe where I should be posting this and/or any directions I should be searching in. Thanks for your assistance.
