Choral Public Domain Library

I've got a handwritten sheetmusic from a Syrian priest. However the problem is, we don't know anything about its origins (is it really St. Ephrem's prayer, if so which one ?, is there a proper English translation, etc... )and possibly scans in Gregorian notation. All i know is, it is sung during lent in eastern churches.

Can someone help ?, I've searched the net for many years.

yea, i've checked that and other sites in non-english , the thing is, i really don't know the original text for this tune. I was expecting someone who actually sung this would pop and help.
